Do they ever perform surgery on piles?

They perform various surgeries on hemorrhoids such as rubber band ligation where in they take the band and wrap it around your piles, hemorrhoidolysis that use electrical cutters or sclerotherapy uses a hardening agent. Though there are home remedies like a hot sitz bath, and some pain relievers.

Yes, if the hemorrhoids are especially bad surgery can be performed. It is usually an outpatient procedure and there are several different types. Alternative treatments are sitting in sitz baths, taking pressure of the area, and being extremely careful when having a bowel movement. These treatments will mitigate the problem.

There is a surgery for piles, but is usually a last resort. Try to regulate the bowel movements to prevent straining which will in turn eliminate the problem. Improve the diet in general and add chia seeds to regulate the elimination. Piles are caused by straining to much for hard bowel movements.

Often piles require surgery for removal. Not only the removal is surgical but the repair may require a plastic surgeon. The plastic surgeon will perform a skin flap procedure.
