Technically, yes, you have to consent for treatment. Even if you are admitted in the hospital, you can walk out if you decide not to get treated. However, you should keep in mind that it will be documented in your medical chart that treatment was offered and refused against doctor's orders.

Yes, you have to give consent to get treatment for an illness. However a family member or even a hospital can petition the court to make the decision on your behalf, if they feel you are making decisions that are harmful and show you are not being responsible. Make sure you sign a living will.

Doctors are required to care for patients in Emergency Rooms. If you do not want care, you require a clear refusal of care letter, a "do not resuscitate" order, and a medical power of attorney naming someone who will agree to your wishes if you are declared incompetent or are unconcious, thus unable to deny care.

You do have to give consent. If you are under the age of eighteen, however, your legal guardian can authorize treatment, even if you don't want it. As an adult, however, doctors have to have your consent before they do anything to you, including taking your temperature or blood pressure.

If you are over 18 and mentally competent, you do not have to be treated without your consent unless you have an infectious disease, such as tuberculosis, or an STC (treatment for these conditions is mandated by the CDC).
