First you can contact the employer and they should still have that information on file, if they don't you can file a form with the IRS and they can give you the W2 information submitted by that employer for your social security number.

Your old employer. Contact your old employer and tell them that you need these forms. They or their accountants will have copies in storage.

The IRS retains copies of all of your old W2 forms. You can simply ask for a transcript of your wage and income information to be mailed to you. This is available for the last 6 years worth of W2 forms. You can call 1-800-829-1040 for more information or to make your reuest

In order to get true, exactly duplicate copy of an old W2 form, it may not be possible. Best place to start is the employer. If employer is no longer in business, go to the IRS. IRS will be able to provide a transcript of earning for year(s) in question will all pertinent info.

You should be able to find old W-2 forms on the internet. You will need to search on "W2" and the year, such as "2004".
