The earrings that they first use are called starter earring and they are a fairly small gauge. You have to work up to the larger ones by replacing the smaller gauges weekly with larger ones.

Yes it is slightly painful, but tolerable it is after all stretching the skin out making a small hole even bigger.

Its not really painful, but it will make your ears sore so be careful how you sleep if your going to keep them in all day.
