Should I be concerned about blood in cat poop?

Probably. Note how much blood there is, any changes in eating or drinking habits, any changes in pet's behavior and call a vet for further evaluation or an appointment.

this could be caused by a few things. It could be internal bleeding, or possible the fecal matter is tearing the anus. Either way, if it persists go to vet.

Bright red may just be a hemorrhoid or a rectal tear. Blood that is dark red to black may be evidence of an ulcer or internal organ damage.

probably it is worm, but check if it has diarrhea also or not.

You should absolutely be concerned! It could be as simple as worms, but could be much serious. See your vet as soon as possible.
