There are thick books with all the details, but try contacting the ADA directly for information specific to your site. Your local planning office would be familiar with the guidelines.

A physician must complete a state-required form stating that the individual requires access to handicapped parking. The form is then carried by the individual to the Dept of Motor Vehicles and a permit is issued. The physician alone must verify the need for consideration. The ADA has nothing to do with it.

Accessible parking spaces should be the spaces closest to the accessible entrance, A sign with the international symbol of accessibility must be located in front of the parking space
