This can be the sign of a false pregnancy. The symptoms occur from a hormone imbalance in the animal, and this can occur after they have been spayed. It is rare though. It is very unlikely that any spayed cat will have this issue barring an illness that would need to be investigated by a vet.

A swollen vulva is not very common when cats are spayed. It could signal an infection, so she should be taken in to see a vet.

Sometimes when a cat is spayed a small portion of an overy can be missed so the cat can still exhibit some signs of being in heat, such as a swollen vulva.
