What do I need to take with me to sign up for medicare?

You will need your birth certificate,social security card ..You should also bring your w-2 forms for the last two years....It would also be a good idea to bring prove of your mailing address..

You will need your birth certificate, your social security number, and information about how much money you have made this year if you are currently employeed. You will need information about your spouse or ex-spouse if divorced (to see if you qualify for any of their benefits).

You will need to call and make an appointment at Social Security. Take your award letter and your proof of income. Take your mortgage papers if you own a home. Make sure that you have your lease or rent reciept if you rent your home. Take documentation if you live with someone else.
