What is the proper etiquette for signing personal correspondence to a doctor?

When writing to a doctor just use regular polite etiquette. I like the phrases "Sincerely Yours" or "As Always". That will be fine.

The all-purpose "Sincerely" would do well. The closing "Yours truly" is more formal, so would be used in business correspondence to the billing office, for instance, but not in a personal letter to the doctor.

I would say that personal correspondence with a doctor is no different to correspondence with any other person, so I would end any correspondence with "Yours truly" or "Yours sincerely."

When sending correspondence to a doctor, as a patient the appropriate signing would be, sincerely yours, respectfully yours, or sincerely. Any correspondence that you have should be business like and professional. Questions regarding your medical history or records would always require the above sign offs. Unless of course your doctor is a personal friend or confidant.

It would be advisable to use formal language and possibly type the document as opposed to hand written. Ensure the correspondence is address to the doctor.

If you are writing to your physician, sign the letter by the name s/he calls you. If you don't know the physician personally, sign your first/last (full) name.
