You really can't avoid them completely. They best way to avoid them is to not reschedule them. Try to buy directly from the airline instead of travel websites and ask for ones that aren't unrefundable. You'll may more up front, but your charges to changes will be lower.

Well you can avoid the charges by stating that there was an emergency. You have to give some kind of proof. Talk your way out of the charges. Tell them you will not fly again.

This depends on several factors, namely who you book with, airlines, and travel companies, and if you purchase travel insurance. Many companies such as price line offer a great deal of flexibility with travel plans. Many airlines also can be flexible but you kinda have to get someone who can be sympathetic.

If you are fairly certain that you will have to reschedule a flight, or if you do not know when you'll be flying for sure, you can buy flight insurance, which is much cheaper than the regular change fee and lets you change the flight for free. You must buy this insurance in advance.
