I use to wonder myself about the cotton in pill bottle, but after working in a pharmacy for several years I finally asked the pharmacist. When the pills are being shipped, they are shook, jostled and I am sure tossed around, so to keep them from being broke or chipped the cotton keeps them in place.

The pills in bottles do not fit snugly all the way to the top of the bottle. Cotton is put into the space between the pills and the bottle top to keep the pills from bouncing around in the bottle and cracking or falling apart while they are being packed and transported.
